PRINT 1 of the ATTACHED FORM for each shipment
STEP 1 - Tick one (for ship to addresses, refer to the bottom of the form). Print 1 per shipment
STEP 2 - Fill in at least 1 of the field below so we know what this shipment is
STEP 3 - Describe parts being returned (faulty parts only)
STEP 4 - Print 3 Copies
STEP 5 – SHIP PARTS to the correct warehouses (see addresses below)
STEP 6 - Email Completed Form and Con Note Copy
